The return of ultra-flat shoes has raised health questions across generations, orthopaedics warn that prolonged use could lead to back pain
From ballet flats and minimalist sneakers to slim-soled loafers, ultra-flat footwear has made a strong comeback in recent fashion trends. While Gen Z has helped popularise the aesthetic through social media and celebrity-inspired looks, these shoes are now being embraced by people across age groups. However, as their popularity grows, orthopaedic specialists are warning that wearing ultra-flat footwear for prolonged periods may come at a cost.
Doctors say they are seeing increasing complaints of foot, knee, hip and lower back pain among patients, although they caution that shoes are only one piece of a much larger health puzzle. Sedentary lifestyles, poor posture, muscle weakness and long hours spent sitting are equally important contributors to musculoskeletal problems.
Flat Shoes Affect Your Back
The connection between footwear and spinal health lies in the body’s alignment. Shoes with very thin soles and little arch support absorb less impact during walking, forcing the feet and lower limbs to bear greater stress. Over time, that strain can travel upward, affecting the knees, hips and lower back. Dr Rakesh Nair, Consultant Knee Replacement Surgeon at Zen Multispeciality Hospital, hembur, says the trend is reflected in his clinical practice.
“We are seeing more young adults coming to the clinic with complaints of heel pain, foot strain, knee discomfort and lower back pain. While ultra-flat shoes are not the only reason, they can contribute to these problems because they provide very little cushioning and arch support. Walking or standing in them for long hours puts extra stress on the feet, which can gradually affect the knees, hips and back,” he explains. However, he adds that footwear should not be viewed in isolation. “Factors such as poor posture, prolonged sitting, obesity and lack of exercise also play a major role in these complaints.”
Not Just a Gen Z Problem
Doctors emphasise that the health risks associated with inadequate footwear are not limited to one generation. Anyone who spends long hours walking, standing or commuting in unsupportive shoes may experience discomfort, regardless of age. Office workers, retail employees, teachers and older adults with existing foot conditions may be particularly vulnerable if they rely on ultra-flat shoes as their everyday footwear.
Dr Chandrashekhar Dixit, Consultant – Orthopaedics and Joint Replacement Surgeon at Jupiter Hospital, Pune, says, “Ultra-flat footwear is not automatically harmful, but prolonged use of shoes with minimal cushioning, inadequate arch support and poor shock absorption can place additional stress on the feet and lower limbs. In some individuals, this may contribute to overuse symptoms involving the feet, ankles, knees, hips and lower back.”
He also cautions against oversimplifying the problem. “It would be an oversimplification to attribute a rise in musculoskeletal pain among Gen Z solely to footwear. Long hours of sitting, poor posture, reduced physical activity, excessive screen time, muscle weakness and increased body weight can also play significant roles. If a person notices recurring pain after wearing a particular pair of shoes, the footwear should be considered as one possible contributing factor.”
Style vs. Comfort
Experts stress that occasional use of ultra-flat shoes is unlikely to cause lasting harm in healthy individuals. According to Dr Nair, consumers do not necessarily have to give up fashionable footwear. “Wearing ultra-flat shoes occasionally is generally not a concern, but using them every day for long periods may increase the risk of foot pain, plantar fasciitis, tendon strain and joint discomfort, especially in people with flat feet or existing foot problems,” he says.
When choosing stylish footwear, look for shoes with good arch support, adequate cushioning, a flexible but supportive sole and a comfortable fit with enough room for the toes. Fashion and comfort can go together, and alternating between different types of supportive footwear can help reduce unnecessary strain on the body.
Matter of Balance
The renewed popularity of ultra-flat shoes highlights how fashion trends can influence everyday health choices. As more consumers embrace minimalist footwear for its sleek appearance, the message from experts is straightforward: style does not have to come at the expense of comfort. Paying attention to proper support, rotating footwear and maintaining overall musculoskeletal health may help ensure that a fashion statement does not turn into a long-term health concern.
