American Oncology Institute Hyderabad Successfully Saves Patient

Hyderabad: American Oncology Institute at Citizens Specialty Hospital in Hyderabad successfully performed advanced external radiotherapy on an 88-year-old male patient suffering with high-risk prostate cancer. The most advanced form of radiotherapy technique, also known as hypo fractionated adaptive radiotherapy, was led by Dr Harjot Kaur Bajwa, Radiation Oncologist and Brachytherapy Specialist along with her team. The patient was declared cancer free at follow up and was in excellent health maintaining his quality of life.

Mr. Batra (name changed) was presented with increased urination and difficulty in passing urine for 3 months. Upon further evaluation, the patient was diagnosed with high-risk prostate cancer (advanced stage). Prostate cancer begins when cells in the prostate gland start to grow out of control.

Local symptoms associated with prostate cancer can include Lower urinary tract symptoms (LUTS), blood in urine, Erectile dysfunction or Urinary retention. The prognosis and treatment options depend on the stage of the cancer (level of PSA, Gleason score, Grade Group, how much of the prostate is affected by the cancer, and whether the cancer has spread to other places in the body.

"Delivery of advanced radiotherapy in prostate cancer is tricky because the prostate gland moves during and in between treatment. Also, the surrounding normal structures like bladder, bowel and rectum are highly mobile and may have different degree of filling every day. This can significantly affect the actual dose delivered to the normal structures and increase the side effects. "Sometimes, extreme filling of the rectum can displace the tumor out of the treatment field, and we may entirely miss the target leading to ineffective treatment," stated Dr Harjot Kaur Bajwa.

She further explained, "Since we are delivering a very high dose of radiation over a short course of time, we need to accurately localise the tumor before each treatment. With the use of daily imaging on ETHOS machine, we are able to clearly assess the position of the tumor and the normal structures. Depending upon the change in anatomy, the machine performs daily online adaptive planning which results in accurate radiation delivery and minimizes toxicity. "
